Who are the ideal candidates for URS?

A patient who has stone in their ureter needs ureteroscopic removal of stones. Not all ureteric stones need surgery the small ones can be removed with medicine and adequate water intake.

Surgery is needed if:

  • Stone is big in size more than 8mm.
  • Medical therapy given for sufficient time has not helped in stone removal.
  • Stone is causing a lot of swelling (hydronephrosis) which in turn can lead to renal failure.
  • A badly impacted stone causes a lot of pain requiring multiple oral and injectable analgesics.
  • A ureteric stone has led to infection and pus formation, leading to high grade fever and flank pain.
  • A ureteric stone causes a lot of bleeding in the urine (haematuria)
Ureteric Stone Surgery
Advantages of URS
  • It is cut less surgery performed using a long and thin endoscope called a ureter scope.
  • Patient experiences less post-operative pain
  • It can be performed on a short stay or daycare procedure
  • Less bleeding as laser is used to fragment the stone
What are the preparations to undergo this surgery?

The ureteric stone surgery in Delhi performed at our facility is decided after thoroughly examining the patient’s situation. Ideally, all patients with a ureteric stone should undergo a non-contact CT scan (NCCT KUB). This tells us the exact location and size of the stone and swelling in the kidney.

  • A urine culture should be done pre-operatively to rule out any infection. In case of infection, antibiotics should be started 3-4 days before the surgery.
  • CBC test to help assess the patient’s fitness for surgery.
  • Pre-operative anesthesia check-up.
  • Assessing the control of sugar and blood pressure if the patient is diabetic or hypertensive.
  • Stopping blood-thinning medicine such as aspirin or clopidogrel at least a week before surgery.
  • Patients are advised to come fasting on the day of surgery.
How is URS done?
  • This procedure can be done under spinal and general anesthesia
  • Patients are placed in lithotomy positions i.e. legs supported in the air.
  • Through the urine passage (urethra) a long thin telescope (ureter scope) is inserted into the bladder. After that under radiological guidance is inserted into the ureter with stone the ureteroscope is then gently introduced into the ureter.
  • The stone after visualization is broken into smaller fragments and removed with special forceps.
  • At the end of the procedure, a hollow tube (called double J or DJ stent ) is placed from the bladder into the kidney
  • The stent aids in decreasing kidney swelling (hydronephrosis) and reduced the risk of small stone fragments from blocking the ureter and causing pain.
What to expect in the post-operative period?
  • Post surgery, the patient is shifted to the room with a urinary catheter.
  • Anti-biotics injections are given to prevent infections.
  • Injectable painkillers are given for pain relief.
  • Patients are given a soft diet once a full recovery from anesthesia happens.
  • Slight discomfort is expected on the day of surgery because of the urinary catheter and urs DJ stenting
  • Patients are discharged after 24 hours of removal of the catheter.
What to expect after discharge?
  • Most patients have an uneventful recovery.
  • For the first few days, patients may notice pinkish urine which clears with time.
  • There may be mild burning sensation and urgency and frequency of micturition.
  • Patients may experience mild discomfort in their back after the passage of urine due to stent.
What are the complications of URS?

Occasionally one finds during the surgery that the ureter is very narrow and it is not possible to safely reach up to the stone in such cases a DJ stent is placed in first silting which will help in widening of the ureter and makes safe access to the stone at a later date. To avoid any complications you should choose the ureteric stone surgery in Delhi at our facility as you will be treated by a skilled doctor, Dr. Niren Rao, who is experienced in handling challenging cases. Visit Delhi Urology Hospital to know how much a dj stent removal cost
